JOBSUMMARY
The Occupational Therapist providesoccupational therapy to patients who require restoration ofphysical and mental functioning in order to gain independence inself-care and other May supervise Certified Occupational TherapyAssistants and QUALIFICATIONS Certificate or degree from anaccredited occupational therapy Licensed by the state in which theOccupational Therapist resides or practices, if state requires, andhas received certification from the National Board forCertification in Occupational Therapy,(NBCOT).

QUALIFICATIONS
Certificate or degree from an accredited occupational therapyPreferably two years of clinical experience (if applicable).Licensed by the state in which the Occupational Therapist residesor practices, if state requires, and has received certificationfrom the National Board for Certification in Occupational Therapy,(NBCOT).

RequirementsPHYSICALDEMANDS
Requires moderately heavyoccupational exertion on a regular and reoccurring basis such asextensive driving, assisting patients in transfer activities(wheelchair to bed, to tub, to commodes) and providing substantialsupport to individuals in ambulation Regularly required to usehands and arms, stoop, kneel, crouch or Occasionally required toOccasionally lift and/or move over 100 Specific vision abilitiesinclude close, peripheral, depth perception and ability to adjust

Observes, records and reports tothe physician the patient s reaction to the therapy program or anychanges in the patient s
Advises, consults and instructs patients in the use of prosthetic,orthotic and assistive Instructs, teaches and supervises otherhealth care team personnel in the case of patients requiringoccupational
May superviseCertified Occupational Therapy Assistant(s) and co-sign CertifiedOccupational Therapy Assistant s progress
Adheres to all applicablefederal, state, local and company maintained standards of care andethics and policies concerned with the practice of occupationaltherapy and among other things, personnel
Participates in staffdevelopment Participates in in-services education
Demonstrates evidence ofcontinuing education conducted for the in-service
Meets productivity andutilization expectations as established by the Company and the
