The Population Health Management (PHM) Population Identification Manager plays a critical role in the identification and stratification of different patient populations within our health system. The main task of the successful candidate is to leverage big data, genetic data, and principles of personalized medicine to identify specific population cohorts for targeted PHM interventions and programs. This role will incorporate a diverse set of data-driven approaches, including predictive modeling, risk stratification, and machine learning, to enhance patient care, improve health outcomes, and reduce healthcare costs. The candidate will be a key player in transforming NEOM healthcare delivery model into one that is proactive, predictive, and personalized, ultimately leading to better health for all our patients.

Key Accountabilities & Activities:

  • Support in the development and delivery of the division strategy through hypothesis development, scenario testing, feasibility studies and related tasks
  • Contribute to preparation of reports and presentations in the annual planning and budgeting cycle
  • As part of the team
  • Collaborate with population health experts to incorporate wellness programs into the broader population health management system and into broader health and wellness initiatives.
  • Reassess and adjust targets and implementation plans accordingly to new and/or changed master plans
  • Support in development of structures and processes for monitoring of projects and activities
  • Independently lead and deliver assigned activities and projects
  • Proactively identify risks and develop mitigation plans
  • Provide periodic reports regarding project progress, challenges, risks, and mitigation plans
  • Actively take part of and contribute to workshops and activities related to asset, equipment and digital infrastructure development
  • Contribute to development of scope of work documents
  • Support in internal and external communications activities
  • Actively participate in sector and cross sector meetings and initiate meeting structures for effective interaction with stakeholders related to assignments
  • Manage stakeholders' feedback on issues, challenges and unmet requirements and develop action plans
  • Act as a role model for the sector in interaction with other sectors and departments
  • Lead by example, supervise and coach team members and colleagues to perform at their best

Background, Skills & Qualifications:

  • Minimum 10+ years of experience in care coordination or healthcare delivery role, preferably within population health management.
  • Min 5 years of experience in healthcare
  • Min 5 years of Project Management experience
  • Strong understanding of healthcare systems and trends, including emerging practices in personalized medicine and genetic data application.
  • Prior experience with Electronic Health Records (EHR) systems.
  • Knowledge of healthcare trends, workflows, and operations.
  • Strong analytical, problem-solving, and decision-making capabilities.
  • A history of successful cross-functional collaboration and change leadership.
  • Understanding of health policy, healthcare reform, and industry standards related to population health management.
  • A bachelor's degree in a healthcare-related field is required, such as Medicine, Dentistry, Public Health, Health Administration, Nursing, or Health Informatics.
  • A master's degree or higher in Public Health, Biostatistics, Healthcare Administration, Nursing, or a related field is strongly preferred.
  • Certification in Health Data Analytics (CHDA) or similar would be a strong asset.
