**Basic Function**:
Senior Pharmacist functions and responsibilities include the following:
- Medication therapy assessment for dosing, allergies, drug-drug interactions, and drug-food interactions.
- Processing, entering and verification of medication orders at specialized units.
- Medication dispensing; ensuring right patient, right drug, right time and right dose and route.
- Conducts day-to-day operations and coordinates on-going work activities.
- Supervises and maintains the activities of one of the Groups/Segments as assigned.
- Serves as a resource person for the assigned specialized area/unit.
- May serve as the primary representative for Pharmacy Services in activities such as committees, taskforces etc.
**Scope**:
This position is responsible for the delivery of pharmaceutical care to patients using the medication use process (selecting, prescribing, dispensing, transcribing and monitoring of medications). Performs comprehensive assessments, analysis, interpret, and recommendations of specific disease management or drug therapy in collaboration with patient care team members or based on approved disease/pharmacotherapy management protocol. Accountable for on-going evaluation and documentation of all components of pharmaceutical care delivered to patients. Must have additional knowledge and training in a selected pharmacy specialty field such QI, IV, TPN, nuclear, hyperlipidemia, Diabetic education, anticoagulation and other ambulatory services. Adheres to The Kingdom pharmacy practice laws and regulations.
**Principal Contacts**:
INTERNAL: Frequent contact with Pharmacy personnel in order to ensure smooth operations. Frequent contact with physicians, dentists and nurses for clarification/advice on specific medication prescription/orders. Occasional contact with other areas such as dietary, maintenance, security etc. for operational needs
EXTERNAL: Occasional contact with patients to solve medication related problems referred by the group/segment staff. Occasional contact may be made within established guidelines regarding borrowing or lending of pharmaceuticals to area hospital and clinics.
**Principal Duties**:
- Provides patient-centered pharmacy services and pharmaceutical care in either a decentralized or centralized manner.
- Provide comprehensive and individualized patient treatment care plan in selected services such as Ambulatory care clinic (Anticoagulation, Medication management), IV and chemo room etc.
- Ensures prompt and accurate pharmaceutical care and services for patients, nurses, and physicians.
- Transcribes, verifies and dispense medication orders in accordance with legal requirements and hospital policy.
- Interprets and evaluates the validity and safety of medication orders and prescriptions and check for appropriate dosage, drug interactions, allergies, and or contra-indications.
- Communicates with the physician regarding appropriateness of a medication order.
- Compounds, packages, labels, and dispenses drugs and intravenous admixture solutions; and confirms accuracy of work performed by technical and pharmacy intern support personnel.
- Act as a liaison with nursing and medical staff concerning prescription orders, drug and pharmaceutical details, patient reactions, adverse drug events and quality improvement concerns.
- Provide timely communication to peers, supervisors and other healthcare professionals to keep them advised of events and problems.
- Precept pharmacy students and interns as needed and train new pharmacists and assistant pharmacists.
- Serve as a resource for any inquiries from healthcare professionals about patients or medications.
- Documents all the interventions in a readily retrievable format.
- Provides medication education and counseling to patients as needed or assigned by the supervisor/designee.
- Ensures adequate stock replenishment on a regular basis including narcotics when assigned.
- Reviews drug usage patterns and suggests controls (ex; revision of ADDS list) when needed.
- Must be able to handle frequent interruptions and adapt to changes in workload and work schedules.
- Ability to set priorities, make critical decisions and respond quickly to emergent situations.
- Maintains required licensure, continuing education requirements and operational competencies as required by regulatory authorities and company policies
- Performs other miscellaneous related duties as requested by the Supervisor.
**Education**:
- Recognized Pharm-D degree or Bachelor of Science Degree in Pharmacy by Saudi Arabia or equivalent. with 3 years of related work experience
- Master's degree/Pharm.D is preferred.
- Minimum of 3-year experience for expatriates (licensure requirements).
**Certifications/Other requirements as applicable**:
- Certification/Competencies in specialized services such as: IV, TPN, QI, Anticoagulation etc.Licensed or eligible to be licensed to practice pharmacy in Saudi Arabia.
