Serves as a pharmaceutical caregiver to specific patient population by efficiently designing, recommending, activating, monitoring, and evaluating patient-specific pharmacotherapy to ensure optimal outcomes under the direct supervision of the respective preceptor. Ensures implementation of the pharmaceutical care plan by assuring the accurate and timely distribution of medications to patients, Serves as a clinical resource for other pharmacy personnel, medical and nursing staff, as well as other healthcare professionals.

**Essential Responsibilities and Duties**:
1. Assists medical staff with the selection of drug therapy and monitoring for a specific patient population.

2. Assumes responsibility for patient-specific pharmacotherapy plan to assure optimal outcomes.

3. Actively participates in specialized patient care rounds with other members of the specialized healthcare team.

4. Participates in patient education wherever required and answers physician's inquiries regarding drugs.

5. Reviews and interprets physician's drug orders for clarity, rationality, dosage form, allergies, interactions, incompatibility, etc.

6. Enters and processes medication orders whenever scheduled to do so by the Head, Medical/Critical Care Pharmacy or whenever the need arises.

7. Determines workload priorities, verifies medication orders, checks the final product for accuracy, and assures timely delivery of the medications to the patient care area.

8. Acts as drug information and education resource for patients, students, colleagues, medical and nursing staff, and other healthcare professionals.

9. Documents therapeutic recommendations in the patient's medical record and pharmacy intervention forms.

10. Detects, analyses, and reports adverse drug reactions.

11. Assures adherence to the departmental policies and procedures and provides monthly reports on clinical activities and other relevant workload statistics.

12. Participates in the performance improvement, educational, and other clinical activities.

13. Follows all Hospital related policies and procedures.

14. Participates in self and other's education, training and development, as applicable.

**Education**:
Bachelor's Degree or Pharm.D. Degree in Pharmacy from an accredited college of pharmacy required.

**Experience Required**:
Sucessful completion of the second year Pharmacy Practice Residency Program (PGY-1 residency).
