Medication Safety Specialist

Develop the Hospital's medication safety plans and organize the overall medication safety activities. Work in concert with Pharmacy, Nursing, and Medical Quality Improvement staff towards process improvements in the medication use system. Facilitate the use of continuous quality improvement and process innovation strategies that support the medication safety strategies.

**Essential Responsibilities and Duties**:
1. Lead the process of the identification and implementation of best practices for medication safety that will support the reduction of medication errors.

2. Participate in the development of hospital wide medication safety and medication errors indicators.

3. Analyses the Hospital's current medication safety practices that contributes to medication errors and take proactive steps for preventions. (ISMP Safe Medication Self
- Assessment for Hospitals).

4. Oversees the management and use of medication error information. Reviews internal error reports and utilizes the information from external reporting programs (i.e. ISMP medication Safety Alerts).

5. Lead and oversee medication-error reports review, investigation to determine root cause, and data entry into database. Review data and determines trends for process improvement, develops the require measures to eliminate the likelihood of occurrence or recurrence of errors.

6. Participates in the designs and development of competency assessments for evaluating the knowledge and ability of healthcare providers to perform tasks related to medication safety.

7. Leads the development and review of all medication-use, medication safety and medication error reporting policies and procedures.

8. Lead and oversee the educational activities of all healthcare staff involved in the medication use process to promote safety medication practices. Develops and supervise the implementation implements the most up to-date educational programs to improve patient and medication safety and regulatory compliance.

9. Review and approve educational presentations that facilitates the understanding and implementation of the medication safety standards within the hospital including High Alert Medications, Look-Alike/Sound-Alike drug names and illegible orders.

10. Serves as resource member on issues related to medication safety. Participates in departmental, multidisciplinary, hospital, health system and regional committees related to medication safety, adverse drug events, medication errors, policy review and development

11. Supports and encourages error reporting throughout the hospital through a just culture approach and participate in the good catch program progress.

**Education**:
Master's or Bachelor's Degree in Pharmacy from an accredited college of pharmacy is required.

**Experience Required**:
six (6) years of related experience with Master's or eight (8) years with Bachelor's Degree of related experience is required.

**Other Requirements(Certificates)**:
Certified professional in patient Safety (CPPS) is preferred
