Job Summary:
The successful candidate will be responsible for performing clinical duties as per assigned clinical privileges, assessing patients thoroughly, and developing plans of care using an evidence-based approach.
Key Responsibilities:
- Clinical Duties: Performs clinical duties as per assigned clinical privileges, ensuring timely completion of tasks and maintaining high standards of patient care.
- Patient Assessment: Assesses patients thoroughly, including clinical, socioeconomic, psychological, and functional assessments, documenting findings in medical records as per DSFH policies.
- Plan of Care: Develops plans of care using an evidence-based approach, documenting in patients' medical records as per DSFH policies.
- Implementation: Ensures timely implementation of plans of care, monitoring progress and making adjustments as necessary.
- MRP (Most Responsible Physician): Becomes MRP for inpatients admitted under their name, developing discharge plans on admission and involving discharge planning services as early as possible if warranted.
- Daily Assessment: Assesses inpatients at least once daily, documenting daily assessment and plan of care in daily progress notes using SOAP format and as per DSFH policies.
- Medical Consultations: Responds to medical consultations in a timely manner, as per DSFH policies.
- Documentation: Documents medical interventions performed on patients in a timely manner, as per DSFH policies.
- Discharge Summaries: Completes discharge summaries in a timely manner, ensuring indicated discharge instructions and education on take-home medications prior to patient discharge.
- Patient Education: Conducts education for patients and their families, engaging them in the plan of care and documenting provided education in patients' medical records.
- Hospital Duties: Attends to hospital duties, responding to calls from the hospital as per departmental/section call schedule.
- Meetings: Attends departmental and general staff meetings, contributing ideas for improving efficiency, productivity, and patient satisfaction.
- Medical Reports: Writes medical reports for patients, as per DSFH policies.
- Teaching Rounds: Contributes to teaching rounds and teaching activities as requested by the chairperson of the medical department.
- Confidentiality: Maintains patient confidentiality at all times.
- Ethics: Complies with all organizational policies regarding ethical business practices, representing DSFH in a positive and professional manner in the community.
- Other Duties: Performs other duties as assigned within the scope of responsibility and requirements of the job.
- Person-Centered Care: Demonstrates compassionate communication skills, providing support and care to patients as 'the patient is the center of our care.'
