Remembrance Hospice is seeking a qualified Registered Nurse to join our team as a Full Time Hospice Case Manager. As a hospice nurse, you will have the opportunity to make a meaningful difference in the lives of patients and their families by providing expert care coordination, patient assessment, and plan development in collaboration with interdisciplinary teams.

Key Responsibilities

  • Develop and implement individualized plans of care that address patients' physical, emotional, and spiritual needs
  • Coordinate patient care, including scheduling appointments, ordering supplies, and managing medications
  • Provide education and support to patients and their families on end-of-life care, pain management, and symptom control
  • Collaborate with healthcare professionals to ensure seamless transitions of care
  • Document patient progress and outcomes in a timely and accurate manner

Requirements

  • Graduate of an accredited school of nursing with a current license in the state
  • One to two years of recent experience in acute care or home health nursing
  • Excellent observation, verbal, and written communication skills
  • Ability to lift, position, and transfer patients, as well as lift supplies and equipment
  • Valid driver's license with reliable transportation
