Description
Monitors quality improvement initiatives. Responsible for monitoring, performing and implements of variety of tasks in clinical nursing, Quality Improvement Plan under the instructions of the QI & Risk Director, to provide a safe and appropriate and effective patient services.
Ensuring that the highest standard of clinical care is maintained by the introduction and implementation of a systemic approach to planning and delivery of nursing care
Ensures that the appropriate records of the patients? progress are maintained according to the hospital policies and procedures.
Assures nursing and ancillary staff implements patient care based upon appropriate standards of care and within respective scopes of practice.
Ensures and proper communication and relationship between all healthcare professionals.
Ensures adherence accreditation standards e.g. JCI ,CBAHI, across all clinical departments and sites by remaining up to date on relevant standards, conducting staff training in conjunction with clinical leadership, developing protocols and procedures as needed, and performing intermittent chart reviews and audits.
Champions the internal event reporting system to improve uptake and use among clinical staff while creating a ?just culture? to ensure staff feel safe reporting adverse and near-miss events.
Conducts Root Cause Analyses for clinical adverse events and near miss events; develops action plans to reduce risk of recurrence; follows the progress of these action plans.
Identifies areas of potential risk and develops systems within the hospital to minimize risk of adverse events or negative quality outcomes
Collaborates with provider team for effective continuum of patient care
Oversee development and monitoring of quality improvement action plansing the PDSA (Plan, DO, Study, Act) model for improvement and closes plans when the stated purpose is accomplished.
Develops and implements training and professional development of clinical support team members
Supervises controls, monitoring practices, procedures, and internal standards to coordinate QI & Risk Department's efforts.
Identify problems that impact on delivery of quality patient services.
Ensures organization wide policies and procedures are up-to-date, controlled and communicated.
Maintain the Policies and Procedures Web portal ensuring up to date posting of approved policies.
Provides educational and technical assistance to committees and departments in meeting QI Plan objectives.
Coordinates data collection, analysis, trending and tracking.
Supports departments and special teams with required development of QI tools and techniques.
Monitor the changes in external standards and put a plan to QI & Risk Director for effective adoption of such changes.
Performs any other duties within the range of his/her competence, as directed by the QI & Risk Director.
**Requirements**:
Minimum of 4 years Quality Improvement experience as a QI coordinator, with 2 years Quality improvement experience in a JCI Accredited hospital.
Bachelor?s Degree in any related field.
