Job Summary:

The Home-care Physiotherapist is responsible for delivering high-quality physiotherapy services to patients in their homes. This role involves assessing, planning, and implementing personalized rehabilitation programs to improve mobility, relieve pain, and enhance the overall quality of life for patients recovering from injuries, surgeries, or managing chronic conditions.

Key Responsibilities:

  • Conduct comprehensive physical assessments of patients in home settings.
  • Develop and implement individualized treatment plans based on patient needs and medical history.
  • Provide therapeutic exercises, manual therapy, and other physiotherapy techniques to restore function and mobility.
  • Educate patients and caregivers on proper exercise routines, posture, and lifestyle adjustments.
  • Monitor patient progress and adjust treatment plans as necessary.
  • Maintain accurate and timely documentation of patient assessments, treatments, and progress reports.
  • Collaborate with physicians, nurses, and other healthcare professionals to ensure holistic care.
  • Ensure compliance with health and safety standards during home visits.

Required Skills and Competencies:

  • Strong knowledge of physiotherapy principles, techniques, and best practices.
  • Excellent communication and interpersonal skills to build trust with patients and families.
  • Ability to work independently and manage time effectively.
  • Problem-solving and critical thinking skills for developing effective treatment plans.
  • Empathy and patience in dealing with patients of varying conditions and ages.
  • Physical stamina and dexterity to perform therapeutic procedures.

Qualifications:

  • Bachelor's degree in Physiotherapy (Master's degree preferred).
  • Valid physiotherapy license or certification in the relevant jurisdiction.
  • Minimum of 2 years of clinical experience, preferably in home-care or rehabilitation settings.
  • Proficiency in using physiotherapy equipment and tools.
  • Basic knowledge of electronic health record systems is an advantage.

Key Performance Indicators (KPIs):

  • Patient Satisfaction Rate: Achieve at least 90% positive feedback from patients and caregivers.
  • Treatment Plan Compliance: Ensure 95% adherence to prescribed therapy programs.
  • Clinical Documentation Accuracy: Maintain 100% compliance with documentation standards and timelines.
  • Recovery Outcomes: Demonstrate measurable improvement in patient mobility and pain reduction within agreed timelines.
  • Visit Timeliness: Complete scheduled home visits on time with less than 5% delays.
  • Collaboration Efficiency: Maintain effective communication and coordination with healthcare teams, measured by timely updates and feedback.
  • Safety Compliance: Zero incidents of safety violations during home visits.

Job Types: Full-time, Contract

Contract length: 24 months
