Medication Safety Coordinator

Participate and support the Hospital’s medication safety plan implementation. Provide medication safety assessments, activities, and educates others on the system-based cause for medical error. Consults with concerned areas and communicates literature-based programs.
**Essential Responsibilities and Duties**:
1. Acts as primary resource for Identification and implementation of best practices for medication safety that will support the reduction of medication errors
2. Develops projects' proposals related to Medication Safety and performance improvement
3. Assist in the design of educational tools for patients to improve medication safety and efficacy.
4. Analyses the Hospital's current medication safety practices that contributes to medication errors and take proactive steps for preventions. (ISMP Safe Medication Self
- Assessment® for Hospitals)
5. Participate in the management and use of medication error information. Reviews internal error reports and utilizes the information from external reporting programs (i.e. ISMP medication Safety Alerts)
6. Prepare monthly, quarterly and annually medication errors report to the hospital administration and hospital committees. displays data in graphs/charts for best utilization of information.
7. Manages and follow-up on medication-error reporting, review, investigation to determine root cause, and data entry into database. Analyses data and determines trends for process improvement, develops the require measures to eliminate the likelihood of occurrence or recurrence of errors.
8. Participate in the development and review of all medication-use, medication safety and medication error reporting policies and procedures.
9. Designs and implements educational presentations that facilitates the understanding and implementation of the medication safety standards within the hospital including High Alert Medications, Look-Alike/Sound-Alike drug names and illegible orders.
10. Supports and encourages error reporting throughout the hospital through a just culture approach and participate in the good catch program progress
11. Coordinate changes needed to meet CBAHI, JCI and other standards of international accreditation organization related to medication management
**Education**:
Master’s or Bachelor’s Degree in Pharmacy from an accredited college of pharmacy is required.
**Experience Required**:
Four (4) years of related experience with Master’s or six (6) years with Bachelor’s Degree of related experience is required.
**Other Requirements(Certificates)**:
Saudi Nationals Only.?
Certified professional in patient Safety (CPPS) is preferred.
