- Participates in the monitoring, education, and coordination of Risk Management and Patient Safety Program related activities for clinical and non-clinical services.
- Assists with the Implementation of the Risk Management, Patient Safety Program, Plans and Initiatives including:

- Conducting Patient Safety Culture Surveys ensuring adequate survey participation, data aggregation and analysis, generation of Hospital-wide Reports and development Action Plans to be presented to the QPS Council, as well as Unit Reports for the Department specific QI&PS Committees.
- Conducting daily review of Safety Events to identify Severity Assessment and corresponding actions to relevant Business Owners to resolve concerns, identifying events that needs escalation to Departmental and Hospital Committees for further review and action, identification of Significant and Sentinel Events that needs intense analysis. Quarterly data aggregation and analysis, and preparation of reports to the QPS Council. Data aggregation, analysis and preparation of Semi-annual reports for Departments/Units.
- Coordinating and Conducting Patient Safety Leadership WalkRounds - scheduling of activities, development of unit profiles, facilitating Rounds, recording concerns, analysis, prioritization and communication of patient safety concerns to relevant Business Owners and ensuring the timely resolution of Unit concerns through follow-up meetings and preparation of periodic reports to the QPS Council.
- Facilitating Sentinel Events reviews and Root Cause Analysis as per Sentinel Events Policies and Guidelines.
- Development of Performance Measures (working towards Data Warehouse/Balanced Scorecard) as it pertains to Risk Management and Patient Safety.
- Participates in the development of Training and Workshops for Risk Management and Patient Safety Concepts such as Failure Mode and Effect Analysis (FMEA), Root Cause Analysis (RCA), Events Reporting including Near Misses and building a Culture of Safety (Just Culture).
- Participates in the development, review/revision within all KAAUH Quality Improvement, Patient Safety and Risk Management programs, plans, activities, databases, indicators, etc.
- Conducts Orientation for QI&PS system training for all new KAAUH staff.
- Participates in simulation scenarios to assess safe readiness for new and modified systems and processes in collaboration with clinical and non-clinical services.
- Assists in data aggregation and analysis, and preparation of quarterly reports for all assigned Risk Management and Patient Safety databases.
- Attends QI&PS Committee meetings in assigned clinical and non-clinical area(s).
- Respects patients and their families to promote for a Patient-Centered Care culture.
- Participates in and supports quality improvement and patient safety activities as an individual or as part of a multidisciplinary team. Performs other duties as assigned or needed. Job Qualifications and Experience

Minimum or Required:

- Bachelor’s degree in Healthcare profession (i.e. Medicine, Nursing, Pharmacy, Hospital Administration) in which 2 years minimum experience should be in a 300+ bed capacity hospital in Quality Improvement Patient Safety and Risk Management.
- 2 years of Experience.

Preferred:

- Master’s degree in Healthcare Quality and Patient Safety or relevant science.
- Certified Professional in Healthcare Quality (CPHQ) or equivalent.
- No Experience Required
